For just $100 per ticket, you’ll have the chance to win a brand-new car, provided by Heartland Chevrolet and Heartland Chrysler, Dodge, Jeep, and Ram. Your purchase not only puts you in the driver’s seat for an incredible prize but also directly supports Liberty Hospital Foundation’s non-profit mission to enhance health, wellness, and care in our community.

Raffle Details:

  • Start Date: Tuesday, December 3, 2024 (Giving Tuesday)
  • End Date: Wednesday, May 7, 2025 (Hospital Week)
  • Drawing Date: Wednesday, May 14, 2025
  • Ticket Price: $100 per ticket
  • INTRO PRICE: For the first 7 days, ticket pricing will be discounted to $90.
  • Grand Prize: A brand-new car! A 2025 Jeep Grand Cherokee L provided to the Liberty Hospital Foundation by Heartland Chrysler Dodge Jeep Ram & Heartland Chevrolet.

How to Enter:

  1. Purchase a Ticket: Tickets are available for $100 each and can be purchased online through the Liberty Hospital Foundation website or at select community events leading up to the drawing.
  2. Recurring Donation: Tickets are also be purchased by pledging a new or additional $10 monthly recurring donation for a minimum of 10 months. This can be provided online through the Liberty Hospital Foundation website or at select community events leading up to the drawing. After the 10 months have concluded, you will continue to be charged $10 per month as a donation to the Liberty Hospital Foundation until you cancel.
  3. Eligibility: The Health on Wheels Car Raffle is open only to legal residents of the United States who are eighteen (18) years of age or older. All raffle ticket purchases must be completed through official entry methods and include the entrant’s full name, address, phone number, and email address. Each ticket must be associated with only one individual, and prizes will be awarded solely to the named individual on the winning ticket. You (or a representative) must claim the winning ticket at the final drawing at the Liberty Hospital on May 14, 2025, or it will be forfeited. Employees of the Liberty Hospital Foundation and their immediate family members (spouses and children) are not eligible to participate or win.
  4. Drawing Details: The drawing will be conducted in two phases. On May 7, 2025, we’ll randomly select 10 tickets during a live event on social media. If your ticket is drawn, you (or a representative) must be present at Liberty Hospital on May 14, 2025, for the final key drawing. Each selected ticket allows the holder to randomly draw a key. After all keys are drawn, participants will take turns pressing the unlock button . The person whose key unlocks the car wins the car!
  5. Ticket Limit: There is no limit on the number of tickets you can purchase—buy more to increase your chances of winning! The entire raffle is limited to a maximum of 3,000 tickets!
  6. Raffle Prizes: We will also draw a $750 gift card to the ELMS Spa and a $500 gift card to QuickTrip. Names will be drawn to select a key. If the person drawn first does not win the car, they will win the second prize, and so on.
    1. One Grand Prize will be awarded to a single raffle entrant and will consist of a 2025 Jeep Grand Cherokee (approximate MSRP: $41,000). The winner will be responsible for all applicable sales tax, registration, title, and insurance fees.
      1. In addition, the winner will receive a check from the Liberty Hospital Foundation of $20,000 towards the IRS Gift tax of 25% and all other sales taxes and other fees associated with the transaction. Any taxes, fees, or other expenses associated with the transaction in excess of $20,000 are the sole responsibility of the winner.
    2. One Second Place Prize will be awarded to one raffle entrant and will consist of a $750 gift card to The Elms Hotel & Spa.
    3. One Third Place Prize will be awarded to one raffle entrant and will consist of a $500 QuikTrip gas card.
    4. All other remaining winners will receive a $100 QuikTrip Gas Card.
    5. All prizes are non-transferable, and no cash substitution will be offered for any prize.
  7. Referral for Free Tickets: Referral for Free Tickets: Refer five unique individuals who each purchase an entry, and you get ONE free entry! You may not refer yourself. There is no limit to the number of free entries. Each referral must be identified by name at the time of the online purchase. If a referral name is not entered on the online form at the time of purchase, it cannot be applied retroactively. Free entries will be generated on Tuesday, May 7, 2025, at 12:00 PM CST.
  8. Gift Tax, Sales Tax, and other Fees: The Liberty Hospital Foundation will pay the IRS Gift tax of 25% and all other sales taxes and other fees associated with the transaction, up to a total of $20,000 in the form of a check to the winner. Any taxes, fees, or other expenses associated with the transaction in excess of $20,000 are the sole responsibility of the winner.

Rules and Regulations:

  1. Raffle Period: Ticket sales begin at 12:00 AM CST on Tuesday, December 3, 2024, and close at 12:00 PM CST on Wednesday, May 7, 2025.
  2. Notification: The 10 ticketholders selected on May 7, 2025, will be notified via phone and email within 24 hours of the drawing. The selected ticketholders must respond within 3 days of notification. If the ticketholder does not respond within the 3-day window, a new ticket will be drawn. Liberty Hospital Foundation will make every good faith attempt to contact the winners.
  3. Prize Redemption: The car must be picked up in person at Liberty Hospital. The winner will be responsible for all actions required to register and drive the car. The second place, third place, and other winners must accept their prize during the final drawing on May 14, 2024. No cash substitutions are allowed.
  4. Refunds: All raffle ticket sales are final. No refunds will be provided except in the case of raffle cancellation.
  5. Odds of Winning: The odds of winning depend on the total number of tickets sold. The raffle is limited to a maximum of 3,000 tickets!

Health on Wheels Car Raffle Official Rules

  1. Claiming and Award of Prizes

To claim a prize, the potential winner must present valid photo identification, complete an Affidavit of Eligibility provided by the Liberty Hospital Foundation, and sign IRS Form W-9. Additional information may be required to verify eligibility. To claim the Grand Prize (car), the winner must also provide a valid driver’s license, proof of legally required insurance, and complete IRS Form W-2G.

Upon verification, the Grand Prize winner will coordinate with the Liberty Hospital Foundation for pickup of the car within 30 days following the drawing date. If the Grand Prize winner fails to take delivery within this period, the prize may be forfeited. No cash redemption or prize substitution is allowed. All other prizes, including second and third-place prizes, will be awarded directly to the winners.

  1. Revocation of Prize

A potential winner must respond to the initial notification within 14 days to claim their prize. Failure to do so, or failure to provide the required documentation and identification, will result in forfeiture of the prize. The Liberty Hospital Foundation may then conduct an alternate drawing to select a new winner. The Foundation is not responsible for communication issues, including email, internet, or technical malfunctions, that may prevent a winner from receiving notification.

  1. Responsibility for Taxes and Other Costs

The Grand Prize winner will also be responsible for title, registration, licensing, insurance, and any maintenance or operation costs associated with the car.

The Liberty Hospital Foundation will pay the IRS Gift tax of 25% and all other sales taxes and other fees associated with the transaction, up to a total of $20,000 in the form of a check to the winner. Any taxes, fees, or other expenses associated with the transaction in excess of $20,000 are the sole responsibility of the winner.

Fees and other costs are the sole responsibility of the winners. Any transportation or travel expenses for collecting prizes are the responsibility of the winners.

  1. Reservation of Rights

The Liberty Hospital Foundation reserves the right to modify rules, adjust prize details, or discontinue the raffle without prior notice. In the event of misconduct, security breaches, or other unforeseen issues, the Foundation may cancel or alter the raffle. If the raffle is discontinued, winners may be selected from existing eligible entries. All decisions by the Foundation regarding the raffle are final and binding.

  1. Disclaimer

In the event that fewer than 1,000 tickets are sold, the advertised car will not be awarded as a prize. Instead, the Liberty Hospital Foundation will hold a 50-50 raffle, in which 50% of the total ticket sales will be awarded as a cash prize to one winner, while the remaining 50% will support the Foundation’s charitable initiatives. The Foundation reserves the right to use discretion in the administration of the raffle and in the distribution of any additional minor cash prizes. All other non-car prizes will remain as initially listed unless otherwise specified by the Foundation.

      6. General Terms

Purchases of raffle tickets are not tax-deductible. By entering, participants agree to comply with these terms and conditions. The Liberty Hospital Foundation reserves the right to disqualify entries that do not meet eligibility requirements or have been tampered with. The Foundation and its partners are not responsible for technical failures, lost or incomplete entries, or typographical errors. Only the number of prizes listed in the official rules will be awarded.

  1. Release of Liability

By participating, entrants agree to release the Liberty Hospital Foundation, its officers, directors, employees, agents, volunteers, and partners from any liability for injuries, damages, or losses incurred through participation in the raffle or use of prizes. Winners accept prizes “as is” and release all parties from responsibility for defects, maintenance, or use. The Foundation will not be liable for events outside its control, including but not limited to natural disasters, government regulations, labor disputes, or technical failures.

  1. Publicity Release

By accepting a prize, winners grant the Liberty Hospital Foundation and its promotional affiliates permission to use their names and likenesses for promotional and publicity purposes without further compensation, unless prohibited by law. Entrants authorize Liberty Hospital Foundation and its promotional affiliates to take photographs of their participation in the event and acceptance of prices, which they agree Liberty Hospital Foundation may use in its promotional materials, marketing, and on social media.

  1. Privacy Statement

Entrants agree that their contact information may be used by the Liberty Hospital Foundation to share news, promotions, and event updates. Personal information will not be sold or shared with unaffiliated third parties.

  1. Warranty Disclaimer

The Grand Prize car is awarded “as is” with no warranties beyond the manufacturer’s standard warranty, if applicable. The Foundation disclaims any other warranties, including implied warranties of merchantability or fitness for a particular purpose.

  1. Applicable Law

These terms and conditions are governed by the laws of the State of Missouri. All entrants are responsible for complying with applicable laws in their jurisdiction. The raffle is void where prohibited by law.

  1. Arbitration of Disputes

Any controversy or claim arising out of or relating to the raffle shall be determined by final and binding arbitration administered by the American Arbitration Association (“AAA”) under its Commercial or, if applicable, Consumer Arbitration Rules and Mediation Procedures. Except as required by law, the parties and arbitrator may not disclose the existence, content, or results of any arbitration without prior written consent of both parties. The arbitrator has no authority to award punitive damages or consequential damages and has no authority to award compensatory damages exceeding $500. The arbitration will be a documents only/desk arbitration, and the decision is non-appealable. The arbitrator, and not any federal, state, or local court, shall have exclusive authority to resolve any dispute relating to the interpretation and applicability of arbitration. The parties waive any right to commence or participate in a class, collective, or representative action or to utilize class-action, collective-action, or representative-action procedures, and the arbitrator has no power to arbitrate any form of a class, collective, or representative proceeding.

By purchasing a raffle ticket, entrants agree to these terms and conditions and to the decisions of the Liberty Hospital Foundation, which are final in all respects.
