Health, Education, Wellness, & Care

We support patients, individuals, families, and employees in the Liberty Hospital community by creating, funding, and promoting programs that improve health, education, wellness, and care.

Liberty Hospital Foundation

The Patient Assistance Fund’s mission is to enhance the overall health of Liberty Hospital patients and reduce unnecessary, expensive hospital re-admissions.

The Impact

The success of the program comes from increasing patient access to affordable life-sustaining medications, insurance, transportation, primary care physicians, and home health resources. Since launching in March 2014, the program serves nearly 200 patients each year at Liberty Hospital – saving them a combined $135,000 per month on prescription costs and empowering them to take control of their long-term health care needs.

Student Assistance

The Student Assistance Program launched in 2016 as a partnership with Liberty Public Schools. The Liberty Hospital Foundation provides vouchers for nursing staff across the district to refer under- or un-insured students to Liberty Hospital Urgent Care for acute health needs. Over 100 vouchers have been funded that provide students with an appointment to assess their health care needs at no cost to their families. Follow-up appointments and ongoing medications are offered at a reduced rate. Student Assistance helps reduce absences and improve the overall health and well-being of children in our community.
