Patient Assistance Program Increases Effectiveness in 2023-2024

LIBERTY, Mo., July 26,2024 – The Liberty Hospital Foundation received the Annual Impact Report from Liberty Hospital’s Care Management team recapping the fiscal year with incredible results.

The Patient Assistance Program was able to assist 1,503 patients and community members in FY 2024 (July 1, 2023-June 30, 2024). This is an increase of 259 patients and community members from FY 2023. Services ranged from Student Assistance, Medication Assistance, DME, Breast Center Imaging, Cardiac Rehab and Transportation.

The Medication Assistance program provided 172 applications with a total patient monthly RX savings of $82,360.57 in FY 2024.

Transportation touches many aspects of a person’s life; adequate and reliable transportation services are fundamental to healthy communities. Transportation issues can affect a patient’s access to healthcare services. These issues may result in missed or delayed healthcare appointments, increase in healthcare expenditures and overall poor health outcomes. Our mission is to make a positive impact on our communities by addressing transportation issues or barriers. We were able to add Uber Health in March of 2023, as a transportation resources for our community. 813 rides were provided by Uber Health in FY 2024. The Patient Assistance Program was able to assist 988 patients with transportation service. This is an increase of 548 patients and community members from FY 2023. This statement deserves repeating, we were able to help 548 more patients with transportation in FY 2024.

The Foundation is proud to support 100% of these services and will continue to serve patients in the Hospital’s community in FY 2025. Each year, the Foundation budgets for roughly $40,000 in Patient Assistance. Thank you to the Care Management team for continuing to utilize donor dollars in the most equitable way, truly making every dollar matter.
